Patient Navigator

Job Summary: The Patient Navigator provides education and emotional support to patients, guiding them through the healthcare system. This role serves as a single point of contact for referring physicians, patients, and caregivers, providing resources and assistance with accessing supportive care services within BCH and in the community.

Principal Duties and Responsibilities:

  • Meet with and call patients and families to review the implementation of care plans. Refer patients to services frequently used in a clinic and specialized services as needed. Explain hospital/community-based resources to patients and families, providing documentation.

  • Assess patients for referrals to resources. Act as a liaison with internal and community resources. Contact hospital and community resources to ensure access to services.

  • Assist patients and families with scheduling multidisciplinary appointments. Review appointment schedules and instructions. Coordinate logistics for plan adherence such as appointment reminders and arranging transportation.

  • Assist patients with applying for insurance. Provide insurance information to care teams to determine eligibility for services.

  • Maintain relationships for accessing community-based resources. Provide outreach and maintain positive working relationships with providers/administrative staff (including physicians, office staff, nurses, and social services staff). Report outcomes of referrals to the referral source.

  • Document patient care and collect required data. Track outcomes and support strategic planning processes. Develop and maintain data.

  • Attend and participate in care planning meetings to prepare patients for discharge or continuing care.

Education:

  • Required: High School Diploma / GED

  • Preferred: Bachelor's Degree in Social Work, Human Services, or other related fields

Work Experience:

  • Required: A minimum of one year of closely related experience

  • Preferred: Experience with the clinic population or work with community resources

Knowledge, Skills, and Abilities:

  • The ability to communicate effectively both orally and in writing and provide empathy in difficult interpersonal situations.

  • Analytical skills to resolve problems requiring the use of basic scientific, mathematical, or technical principles.

  • Fluency in Spanish or Haitian Creole may be preferred.

Physical Requirements:

  • Frequent lifting and carrying objects weighing up to 10 pounds.

  • Regular sitting, stooping, and bending.

  • Regular reaching and grasping objects at, above, and below shoulder level.

  • Regular grasping and fine manipulation with hands.

  • Regular proofreading and checking documents for accuracy.

  • Regular inputting/retrieving words or data into or from an automated/computer system.
