The Hematopoietic (Stem) Cell Transplant Program at Dana-Farber/Boston Children's Cancer and Blood Disorders Center is one of the largest and most experienced pediatric stem cell transplant programs in the world. Our expertise includes that supporting standard approaches to bone marrow and peripheral blood stem cell transplant as well as novel cellular therapies from stem cells and non-stem cells requiring expertise in graft manipulation, cell selection, and genetic manipulation. Our program integrates the complex inpatient and outpatient care that children and young adults requiring cellular therapies require. Our team includes some of the world’s most experienced and knowledgeable pediatric stem cell transplant doctors and scientists, as well as nationally recognized specialists in other disciplines. Our team includes both inpatient and outpatient advanced care practitioners, bedside nurses, nurse navigators, nutritionists, social workers, psychologists, psychiatrists, and coordinators all with transplant and cell therapy specific expertise.

We care for patients with both malignant and non-malignant disease and we have deep expertise in the delivery of a broad variety of cellular therapies including everything from more standard stem cell transplants to novel genetically modified cellular therapies. Our patients come from a broad geographic area to receive the specialized care that we deliver and as a result require coordination of their complex needs both locally within the Longwood Medical Campus and with providers closer to their homes.

The Patient Navigator:

  • Meets with/calls patients and families to review the implementation of care plans. Refers patients to services frequently used in a clinic and specialized services frequently used in a clinic. Explains hospital/community- based resources to patients and families, providing documentation.

  • Assesses patients for referrals to resources. Acts as a liaison with internal and community resources. May contact to hospital and community resources to ensure access to services

  • May assist patients and families with scheduling multiple-disciplinary appointments. Reviews appointment schedule and appointments instructions. Coordinates logistics for plan adherence such as appointment reminders and arranging transportation.

  • Assists patients with applying for insurance. Provides insurance information to care teams to determine eligibility for services.

  • Maintains relationships for community-based resources. Provides outreach and maintains positive working relationships with providers/administrative staff (physicians, office staff, nurses, social services staff. Reports outcomes to referral source

  • Documents patient care and collects required data. Tracks outcomes and supports strategic planning processes. develops and maintains data.

  • Attends and participates in care planning meetings to prepare patients for discharge or continuing care.

    To qualify, you must have:

  • A High School Diploma / GED, Bachelor's degree in Human Services, or a related field is preferred

  • A minimum of one year of closely-related experience;

  • Experience with the clinic population or work with community resources is preferred.
