The Heart Transplant - LVAD nurse coordinator reports to the Medical Director and Nursing Director of the Advanced Heart Failure Program at BIDMC. The LVAD/Heart Transplant (OHT) Program is a subspecialty within the Advanced Heart Failure Department and is designed to offer the most advanced care to this specific patient population. The registered nurse will be instrumental in the operation of the LVAD/OHT Programs providing comprehensive clinical support and care coordination.

Coordinates care for patients receiving treatment in a specialty service, including the evaluation, assessment, scheduling, consultation and treatment of patients in order to implement a seamless progression of care to meet the patient's needs.

Job Description:

Principal Duties and Responsibilities: (General) Emphasis is on collaboration with Heart Failure physicians, nurse practitioners, staff surgeons, house staff, hospital staff, and the patient for coordinating all aspects of pre-surgical evaluation and scheduling. The nurse will also coordinate patient care with outside cardiologists, other referring physicians, and other facilities. The nurse is responsible for continuity and flow of the patient through the system, from the initial visit to pre procedure assessment, post-operative period on cardiac floor and experience through outpatient visits. This nurse coordinator is also responsible for participating in programmatic development of the above programs as well as participates in Joint Commission certification/re-certification.

Department Specific Job Responsibilities: Advanced Heart Failure/ Cardiac Transplant Program

  • Provides direct nursing care to patients in the Advanced Heart Failure Program which includes LVAD/OHT patients in collaboration with cardiothoracic surgeons and Heart Failure cardiologists and nurse practitioners, house staff, and hospital staff.

  • Helps coordinate care for Heart Failure patients to provide safe, effective patient centered care that meets the needs of patients and families. Oversees consultative and ancillary testing scheduling for patients in the pre- peri- and post-operative phases of LVAD implantation or heart transplantation.

  • Participates in patient rounds and case conferences.

  • Maintains and follows patient care protocols.

  • Performs telephone triage: answers patient telephone calls, both pre- and post-procedure from patients, referring providers, and outside facilities (hospitals, rehabilitation centers, physician offices).

  • Participates in in-patient staff education including nurses, house staff and hospital staff. Responds to requests for in-service education

  • Assists with ongoing feedback to referring physicians in network with regard to patient evaluation and disposition.

  • Provides patient education specific to LVAD/OHT, assesses comprehension of knowledge, and collaborates with multi-disciplinary team members to ensure adequate self-care upon discharge.

  • Continually updates clinical knowledge and skills through consultation, formal and informal education, and review of literature.\

  • Independently reviews and prepares ancillary study results for physician or nurse practitioner analysis.

  • Participates in the Cardiac Transplant and Ventricular Assist Device Program Quality Improvement Processes.

  • Assists the organization in achieving and maintaining Joint Commission certification as a VAD center for bridge to transplant and destination therapy.

  • Maintains all case report forms, patient research records and complies with all Institutional Review Board (IRB), study sponsor, and Food and Drug (FDA) requirements in order to conduct clinical research.

  • On-call responsibility for LVAD and transplant patient emergencies as well as transplant organ call

  • Participates in outreach initiatives to provide formal/informal education as needed to rehabilitation centers, and first responder agencies.

Essential responsibilities including but not limited to:

  1. Collaborates with physicians and other health care providers to coordinate the care for patients from referral/consultation to discharge /follow up. Assists with pre-authorization process.

  2. Serves as contact and facilitator to receive consults and patient information. Coordinates and ensures all patient scheduling including but not limited to consultation, diagnostic and treatment procedures.

  3. Gathers information for history and physical. Performs and documents evaluations and assessments of patient. Assists in ordering labs, x-rays, and diagnostic procedures.

  4. Works in collaboration with surgeons and physicians in educating the patients and their families in medical care. Assists in development of patient/family educational materials. Participates in patient rounds and case conferences.

  5. Updates clinical knowledge and skills through consultation, formal and informal education, and review of literature for self and clinical team. Provides leadership for process improvement related to the care of these patients.

  6. Makes and answers patient phone calls, both pre and post treatment/procedure, from patients referring providers, and outside facilities (hospitals, rehabilitation centers, physician offices).

Required Qualifications:

  1. Associate's degree in Nursing required. Bachelor's degree in Nursing preferred.

  2. License Registered Nurse required., and Certificate 1 Basic Life Support required.

  3. 1-3 years related work experience required.

  4. American Heart Associate - Basic Life Support Certificate

  5. Experience with computer systems required, including web based applications and some Microsoft Office applications which may include Outlook, Word, Excel, PowerPoint or Access.

Preferred Qualifications:

  1. Prior experience in specialty service preferred.

Age based Competencies: Employees in this job must be competent to provide patient care to the following age groups: Neonatal:Birth to 6 months, Youth: 6 months to 16 years, Young adult: 16-30 years, Middle Age: 30 - 60 years, Elderly: 60 - over.

Social/Environmental Requirements:

  1. Work requires close attention to task for work to be accurately completed. Intermittent breaks during the work day do not compromise the work.
