State of Massachusetts Clinical Social Worker (A/B) in Boston, Massachusetts

The Lemuel Shattuck Hospital (LSH) is seeking a Clinical Social Worker (A/B). The incumbent will provide clinical casework and consultative services to a complex community medical population as well as House of Correction (HOC) inmate patient population in accordance with the policies and procedures of Lemuel Shattuck Hospital (LSH), and Department of Correction (DOC) and Department of Public Health (DPH) as well as in accordance with ethics and standards of the staff member’s discipline. Formulate a treatment plan to enable patients to address their emotional and environmental stressors on a medical psychosocial basis. Actively contribute to discharge planning and interdisciplinary team meetings, interagency conferences and collaborating with other public and private agencies in making expeditious referrals. /_Hours:_/ Monday-Friday 8:30-5PM with Saturday and Sunday Off The nature of this job requires the incumbent to occasionally work flexible hours outside of the designated shift. /_Duties and Responsibilities (these duties are a general summary and not all inclusive):_/_ * Primary Clinician for an assigned caseload of community and/or HOC inmate medically complex inpatients throughout their hospitalization. * Provide timely and accurate psychosocial assessments according to LSH and TJC standards. * Develop and coordinate disposition plans in conjunction with community patient, their supports, and treatment team, taking into account developmental, racial, cultural and ethnic, gender, sexual orientation and gender identity issues that impact the patient’s aftercare needs. * Support patient discharge activities through identification of patient discharge options, linkage and referral of patients to aftercare resources and coordination with community providers. * Provide time-sensitive crisis intervention, safety crisis plans, and psychotherapeutic services to community and inmate patient populations. * Complete timely & accurate documentation in the electronic medical record according to LSH, DOC, and TJC standards and policies. * Gather and utilizes clinical data to aid in development of the ongoing psychosocial assessment and discharge planning process. * Clinical documentation includes but is not limited to initial psychosocial assessments, patient contacts, response to treatment, discharge planning, SNF searches, crisis safety plans. * Provide consultation and education to patient and their supports as appropriate to enhance patient’s discharge. * Serve as liaison between LSH and other public and private agencies and collaborates with community providers in the continuum of care as per TJC standards regarding discharge planning, guardianships, legal status, entitlements. * Actively participates in regularly scheduled Rounds, Team meetings, case conferences, and department meetings. * Participates in Social Work Addiction Department Performance Improvement activities. * Continuously assesses and evaluates patient progress and revises treatment plan accordingly. * Assume responsibility for own continuing education and professional growth of others. * Contribute to the enhancement of the therapeutic milieu by facilitating psychoeducational groups to patients and/or their supports utilizing psychotherapeutic group techniques. * Operate computer software programs effectively, particularly Microsoft Word, Excel and Outlook, in order to maintain records, data bases, and communications. /*Required Qualifications:*/_ * Thorough knowledge of and demonstrated ability in psychotherapy techniques: including diagnostic assessment and crisis interventions, individual, family and group work therapy. * Understands the social, emotional, and experiential causes of social and psychiatric disorders. * Experience with addiction, chronic illnesses, disabilities, and/or end-of life care. * Knowledge of community resources and how to access them for patient aftercare and discharge planning with the ability to initiate discharge planning upon admission. * Develop and implement effective clinical interventions with a clinically complex, neurological, and physical challenging patient population. * Ability to interpret and coordinate treatment plans among a variety of human service personnel and delivery systems. * Can establish and maintain professional working relationships with others, and to establish rapport and interact with people who are under stress and to handle stressful and emergent situations. * Utilize supervision effectively, i.e. the capacity for self-examination with specific reference to how one’s own behavior impacts upon patients and colleagues. * Ability to understand, explain, and apply the provisions of the laws, rules regulations, policies, procedures, specifications, standards, and guidelines governing assigned unit activities. * Will exercise discretion in handling confidential information in compliance with hospital standards and HIPPA regulations. * Communicates efficiently both verbally and in writing. * Knowledge of the types and application of standard office filing systems. _Preferred Qualifications:_/ / * Knowledge of policies, procedures and regulations of the department and the Lemuel Shattuck Hospital and how they impact the clinical social worker, patient, and other staff. (Safety, infection, disaster, etc.) * Provide knowledge for critical job elements through orientation and in-service training to keep staff current with respect to regulations and procedures governing such work activities. * Identify, assist, and encourage staff participation in continuing education program for licensing and acquiring knowledge skills, and abilities that may be required by the needs of the patient or clinical social worker. * Familiarity with Microsoft Office Suite computer programs. /_About the Lemuel Shattuck Hospital:_/ Lemuel Shattuck Hospital is a fully Joint Commission accredited teaching facility that provides acute, subacute, and ambulatory care to patients referred by public agencies and private sector providers. Shattuck Hospital’s services help economically and socially disadvantaged patients to get high quality, cost-effective care from a staff that respects their dignity. Located in the heart of Boston’s Jamaica Plain neighborhood, the Shattuck Hospital includes 248 inpatient beds including 95 Psychiatric beds and 29 Correctional Health beds. In addition, the Shattuck has a full outpatient clinic with many specialties that offers treatment to patients of the Department of Mental Health as well as Department of Correction and House of Correction inmates and community patients. The Shattuck hospital is on the forefront of substance use disorder treatment including a Suboxone clinic serving 200 patients.
