FIRSTeam Therapeutic Family Mentor

Job Description:

Sign-on bonus up to $3,000 (prorated for part-time)

The FIRSTeam Therapeutic Family Mentor is a qualified paraprofessional providing case management and skill-building services as part of a two person team to families referred for support and stabilization via a contract with the Department of Children and Families (DCF). The Mentor works with the MA level FIRSTeam clinician to provide services via a team approach over a short term treatment episode lasting an average of 3-6 months. Goals are identified and progress is reviewed in a collaborative process between the family, DCF and FIRSTeam.

Responsibilities;

  • Provides structured one-to-one, strength-based support services to eligible youth and caregivers for the purpose of addressing daily living, social, safety and communication needs.

  • Attendance at minimum weekly 1:1 supervision and other supervision/meetings as requested; checks agency email daily and is available by phone during the day; keeps essential program tracking logs up to date; submits required paperwork as scheduled and requested; conducts satisfaction surveys as needed.

  • Carries a caseload to provide counseling and mentoring, multiples times per week, as assigned by the Department of Children and Families in conjunction with a licensed clinician and delivers services via a team approach. Accommodates emergency referrals from DCF by supporting outreach within 24 hrs of receipt of referral.

  • Collaborate, as appropriate, with DCF, other providers and natural supports of families served.

  • Must uphold and practice a strength-based, trauma-informed perspective when working with youth and families

  • Afternoon/evening hours are required. Situations may arise where staff may need to schedule and hold appointments on weekends based upon the needs of the families served. Exempt staff are required to be available by telephone from 8 a.m. to 8 p.m. during the business week.

  • Other related duties as assigned.

  • ASSOCIATE-LEVEL COUNSELORS/PARAPROFESSIONAL:o Associate-level counselors/paraprofessionals must have an associate’s degree in a human services field from an accredited academic institution and one (1) year of relevant experience working with families or youtho If the associate’s degree is not in a human services field, additional life or work experience may be considered in place of the human services degreeo Associate-level counselors/paraprofessionals are supervised by a licensed clinician with one of the following credentials: LICSW, LMFT, LMHC, LCSW, LADC I, Psychologist, Psychiatric Nurse, or Nurse Clinical Specialist•BACHELOR – LEVEL COUNSELORS/PARAPROFESSIONAL:o Bachelor-level counselors/paraprofessionals must have a bachelor’s degree in a human services field from an accredited academic institution and one (1) year of relevant experience working with families or youtho If the bachelor’s degree is not in a human services field, additional life or work experience may be considered in place of the human services degreeo Bachelor-level counselors/paraprofessionals are supervised by a licensed clinician with one of the following credentials: LICSW, LMFT, LMHC, LCSW, LADC I, Psychologist, Psychiatric Nurse, or Nurse Clinical Specialist
