State of Massachusetts Habilitation Coordinator I in Baldwinville, Massachusetts

The Habilitation Coordinator will coordinate with the residential homes in TCS (Templeton Community Services) Programs. This candidate will need to have an understanding of community integration. This position would look to increase individual and staff knowledge of opportunities within the community. Have the ability to assess the needs of the individuals by building trust and providing non-judgmental support to link them to their preferred activities to facilitate creating meaningful relationships. Provide continued guidance and oversight throughout the process and fade involvement once staff and individuals are confident but remain as a resource. Support staff in coming up with in home, meaningful activities for those who are retired or lack a structured work/day program. Candidates must have effective communication skills, a strong understanding of person-centered approaches, and a commitment to promoting inclusivity and empowerment are essential for success in this position. Candidates must be sensitive to the social/emotional needs, various disabilities, sensory considerations and cultural awareness of individuals being supported by TCS, while placing an emphasis on community integration and individualized activities identified by the individual and/or team members. Provide training for staff and individuals to utilize available resources to promote community involvement. * * Duties and Responsibilities(these duties are a general summary and not all inclusive): * Acts as a liaison between Community Residential Services and Community Organizations, social services agencies, groups and individuals to educate staff and the community on the philosophy of the program and ensure positive membership * Participates in the evaluation, implementation, and monitoring of integration plans, as requested/assigned, to promote consistency and to model implementation for other staff. * Participates as an ISP team member, completes necessary documentation and attends team meetings as appropriate/assigned. * As assigned, provides value-based training to residential and clinical staff on a regular basis. Attends related workshops and conferences to maintain and increases professional development. * Facilitate person centered planning sessions for identified individuals. A Bachelor's degree or higher in Human Development/Human Services, Psychology, Rehabilitation, Special Education, Vocational Rehabilitation Counseling or a closely related field may be substituted for the required (B) experience.
