South Middlesex Opportunity Council Case Manager in Ashland, Massachusetts

SHADOWS

Shadows provide temporary emergency housing for up to 10 women. It is linked to SMOC's continuum of housing and services through the Common Ground Resource Center. Women at Shadows are eligible for employment and housing search services, mental health and substance abuse services and more.

SUMMARY

Provide intensive case management services to single adults residing at the Shadows Shelter. Provide service needs assessment and work with each client to develop an Individual self-sufficiency Plan with the primary goal of enhanced self-sufficiency and a move to a more stable housing outcome-based service.

PRIMARY RESPONSIBILITIES

  • Provide case management services for clients at the emergency shelter.

  • Establish a service plan with each individual.

  • Conduct time limited follow up and support as the client accesses the new services and initiates action plans for change.

  • Provide related information and referrals to community-based resources including Fuel Assistance, Food Stamps, food pantries, and other services that can assist the client in the maintenance of sustainable housing.

  • Engage all clients by understanding and addressing their needs whether within or outside the scope of work.

  • Maintain accurate client files and records; complete required paperwork.

  • Attend & participate in engagement team meetings as requested and communicate effectively with clients and staff in other areas.

  • Maintain confidentiality of client, employee and agency information in accordance with federal and state laws and funder requirements.

  • Fill in required shifts in emergency coverage situations.

KNOWLEDGE AND SKILL REQUIREMENTS

  • Bachelor's degree or a minimum of 3 years’ experience in related field.

  • Sensitivity to low-income people of diverse backgrounds; excellent listening skills.

  • Ability to work independently.

  • Good written and verbal communication skills.

  • Bi-lingual preferred.

  • Must have valid driver’s license, reliable transportation and meet insurance standards.

PHYSICAL REQUIREMENTS

  • Ability to drive a vehicle.

  • Must be able to climb stairs in residences.

ORGANIZATIONAL RELATIONSHIP

  • Directly reports to Program Manager.

WORKING CONDITIONS

As part of the responsibilities of this position, the Case Manager will have direct or incidental contact with clients served by SMOC in various programs funded or administered through the Executive Office of Health and Human Services. A successful background check is required.
